Low-Renin Hypertension accounts for 20-30% of Hypertensive Patients

About

Low-renin hypertension accounts for 20-30% of hypertensive patients and includes monogenic, secondary and essential forms, the most common being low-renin essential hypertension and the next most common is primary aldosteronism. Low-renin and low-aldosterone is rare.  It is usually associated with normal or low BP, so hypertension in this subgroup is very uncommon and should be further evaluated for an secondary cause.

Low renin HTN is the subset of Hypertension that is usually salt sensitive and in general responds best to salt restriction, diuretics and Calcium Channel Blockers (CCB).  Multiple factors should be considered in the differential diagnosis of low renin HTN:

1. Salt- excessive dietary sodium intake may create this functional phenotype in low renin essential hypertension now called low renin primary hypertension

2. Drugs- Low renin and or low aldosterone levels can be drug related. Drugs that reduce the activation of the renin-angiotensin-aldosterone system (RAAS) include beta-blockers, direct renin inhibitors (Aliskerin), adenosine, and centrally acting agents such as clonidine and alpha-methyldopa.
